According to a judge of a TASVideos submission (https://tasvideos.org/6524S) of the hack in 2019: "This game appears to reuse many levels from the original SMB1, only providing minor changes and cosmetic updates."
I'm not sure how else to put it, so make of it what you will, I guess.

Original author is unknown aside from... "MYSTERY!".
